Required: Parent/Guardian Discipleship Participation Consent 

I am the parent or legal guardian of the named minor(s). I grant permission for them to participate in the Building Balanced Teens virtual discipleship sessions and small-group mentor discussions stewarded by the Next Generation Discipleship Council

Required: Child Safeguarding & Online Conduct Agreement 

I agree to provide a safe and suitable space for my child during online sessions. I will help my child follow respectful online behaviour. I understand that at least two approved leaders will supervise each virtual room and that sessions will be monitored and recorded for safeguarding purposes.

Optional: Internal Celebration / Ministry Highlight Media Consent

I permit non-identifiable group screen captures or participant work to be featured in internal cohort graduation celebrations and approved ministry summaries. (You may decline without impacting programme admission).
